Qualification and Achievement requirements for the bidder.
投標(biāo)人須提供以下文件
The following documents must be provided.
（1）投標(biāo)人須按招標(biāo)文件第一冊(cè)第四章格式IV-9.資格證明文件格式的要求填寫(xiě)和提交有關(guān)資料；
All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV and provide relevant information；
（2）投標(biāo)產(chǎn)品原產(chǎn)地在關(guān)境內(nèi)的投標(biāo)人必須是制造商（不接受代理商投標(biāo)）。
Bidder must be the Manufacturer if the goods are originated from PRC
投標(biāo)產(chǎn)品原產(chǎn)地在關(guān)境外的投標(biāo)人必須是制造商（不接受代理商投標(biāo)）；
Bidder must be the Manufacturer itself if the goods are originated from outside of PRC；
(3) 投標(biāo)人須提供各種（合格有效的）資質(zhì)證書(shū)的復(fù)印件：如營(yíng)業(yè)執(zhí)照、經(jīng)營(yíng)許可證等。
Bidder should provide the printed version of various valid qualification certificates, such as business license and operation license, etc (The copy must be signed or sealed by the bidder).
（4）投標(biāo)人須提供開(kāi)戶銀行在開(kāi)標(biāo)之日前三個(gè)月內(nèi)開(kāi)具的資信證明原件或該原件的復(fù)印件（由投標(biāo)人代表簽字或蓋章）。
The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
（5）第八章中要求的其他合格的資格證書(shū)。
Other Qualification and Achievement requirements stipulated in Chapter VIII.。
